Job summary

LiveWell Homecare Agency seeks a licensed Physical Therapist to provide primary PT in patients' homes across a designated five-county Georgia region, supervising PTAs and Home Health Aides and coordinating with multidisciplinary teams to optimize outcomes.

You will manage patient care from admission through discharge, tailor therapy plans, educate patients and caregivers, ensure regulatory compliance, and document accurately while contributing to quality improvement and ongoing professional

Qualifications

• Graduate of an accredited physical therapy program with licensure as a Physical Therapist.
• Minimum of 1 year of full-time physical therapy experience, preferably in home health.
• Current BLS certification and valid Georgia driver's license with private vehicle insurance.
• Proficiency with computer systems and home health software; knowledge of regulatory processes (OASIS preferred).

Role Overview:

This position involves providing primary physical therapy care in patients’ homes within a designated five-county area, including rural locations. The therapist supervises PTAs and Home Health Aides, develops and adjusts care plans, and collaborates with multidisciplinary teams to ensure positive patient outcomes.

Key Responsibilities:

• Assess, coordinate, and manage patient needs according to physician orders, from admission through discharge.
• Perform routine and complex therapy procedures tailored to individual patient requirements.
• Supervise and review the care provided by PTAs and Home Health Aides, ensuring adherence to care plans.
• Educate patients and caregivers to promote self-care and independence.
• Communicate patient progress and changes with physicians and team members.
• Ensure compliance with regulatory guidelines, complete documentation accurately and timely, and participate in quality improvement activities.
• Contribute to staff education, participate in performance activities, and support ongoing professional development.

Qualifications & Skills:

• Graduate of an accredited physical therapy program with licensure as a Physical Therapist.
• Minimum of 1 year of full-time physical therapy experience, preferably in home health.
• Current BLS certification, valid Georgia driver’s license, and private vehicle insurance.
• Strong assessment, rehabilitation, communication, organizational, and problem-solving skills.
