Day Kimball Health seeks a Physical Therapist to join our Homecare team in Thompson, CT. The role focuses on evaluating and treating patients in home settings, with a strong emphasis on independence and goal-driven care.

Ideal candidates have experience in home health or orthopedics within fast-paced clinical environments. A Connecticut license, CPR certification, and a valid driver’s license are required, with opportunities for growth and development.

Qualifications

• B.S. Degree from an accredited school of physical therapy is required.
• Connecticut license or license eligibility is required.
• CPR certification is required.
• Minimum of one year experience preferred.
• Valid Connecticut driver’s license and proof of insurance required.

Responsabilités

• Evaluates and treats patients as prescribed; documents progress in electronic chart.
• Educates patients and families about conditions, treatment, and home program.
• Directs other health care team members as needed.
• Reviews referrals and determines PT treatment required.
• Plans written treatment goals and timelines based on evaluation.
• Attends case conferences when necessary.
