Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled home-based physical therapy services. You will evaluate patients, design treatment plans, and use modalities to improve mobility and independence in daily living.

You will drive to patient homes, adhere to the plan of care, and educate families and care teams while working under the Branch Director's supervision. Requires a valid PT license and reliable transportation.

Qualifications

• Must hold a valid Physical Therapist license in the state of employment.
• Must possess a valid driver’s license and auto liability insurance.
• Must have dependable transportation and be able to drive in various weather conditions.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Aims to improve mobility and independence in daily living for clients.
• Deliver skilled care per physician orders and established plan of care.
• Educate the family and other health care team members.
• Work under the direction of the Branch Director.
