Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled therapies in clients’ homes and oversee care plans. The role emphasizes improving mobility, independence, and activity tolerance, with family and caregiver education included.

Travel to patient homes is required, with supervision by the Branch Director and adherence to physician orders. Candidates must hold a state license and valid driver's license with automobile insurance coverage.

Qualifications

• Must be a licensed Physical Therapist
• Must have a valid state driver’s license
• Must possess automobile liability insurance
• Must wear seatbelts at all times while driving

Responsibilities

• Examine and treat patients with physical impairments using physical modalities
• Aim to improve mobility and independence in daily living
• Administer skilled care per physician orders and care plan, under supervision of Branch Director
• Educate families and health care team members on care and home safety
