Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to evaluate and treat patients in the home setting, focusing on mobility, independence, and functional goals. You will work within specialty programs to deliver skilled services per physician orders and educate families and care teams.

The role requires an active state license, a valid driver’s license, and reliable transportation, with driving in all weather conditions.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Aim to improve mobility, independence in self-care, and daily living skills.
• Participate in specialty programs like Balance and Fall Prevention, Spine Safety, and replacement programs.
• Administer skilled care per physician orders and plan of care, under supervision.
• Educate family and health care team members as part of care delivery.
