Job description

Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time position performing home health visits in the Baltimore, MD area, specifically serving adult and geriatric clients in the Perry Hall territory.

As a home care Physical Therapist, you will be a key part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments.
3. Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and family members on rehabilitative activities to promote health, safety, and independence.
6. Document observations and interventions accurately using a state-of-the-art tablet.

Qualifications:

1. Current Maryland Physical Therapist (PT) license.
2. Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
