Job summary

Remedy Home Health Care in East LA County is seeking a licensed Physical Therapist (PT) to join our Home Health team on a Per Diem / PRN basis. You will deliver skilled therapy in home-based, assisted living, and community care settings to seniors and geriatric patients.

As a PT in home health, you’ll focus on fall prevention, mobility, and independence, with flexible scheduling, mileage reimbursement, and a collaborative clinical environment with nurses, case managers, and physicians.

Qualifications

• Licensed Physical Therapist (PT) in California required.
• CPR certification (AHA approved).
• Minimum 1 year of clinical experience; home health preferred.
• Experience with geriatric or senior care is preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform in-home PT evaluations and reassessments.
• Develop and implement personalized rehab plans.
• Improve strength, balance, mobility, and independence.
• Provide therapy in home health, assisted living, and community settings.
• Educate patients and families on fall prevention and safe mobility.
• Document visits using EMR (WellSky/Kinnser).
• Collaborate with interdisciplinary home health teams.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.
