Adoration Home Health in Rutherford, TN is seeking a dedicated Home Health Physical Therapist to join our team. You will provide patient-centered, 1:1 care to individuals in their homes, evaluating and treating according to physician orders.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence.
• Maintain communication among the treatment team, physician, patient, and family members.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities.

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
