Job summary

Adoration Health is seeking a Physical Therapist for Home Health on a PRN schedule to serve patients in Rutherford, Union City, and Martin, Tennessee. The role focuses on high-quality, patient-centered care in home settings, with team support and strong independence.

PRN shifts offer flexible scheduling and stability within a growing home health team. The ideal candidate will have a PT degree, active license in Tennessee, CPR certification, and at least two years of relevant experience.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid license in the state of practice.
• Current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to advance independence and function.
• Maintain communication with treatment team, physician, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in accordance with state practice act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
