The Physical Therapist provides services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries or disease. The work assignments are varied and often require independent interpretation and decision-making.

As a Home Health Physical Therapist, You Will:

1. Plan and administer prescribed skilled physical therapy treatments for patients with injuries, illnesses, and disabilities to attain optimal physical function.
2. Test and screen patients’ physical strengths to assist physicians in evaluating their level of function and develop treatment plans.
3. Develop and implement conditioning and rehabilitation programs aligned with physician’s plans and patient goals, adjusting as needed.
4. Collaborate with physicians and clinical team members to refine treatment plans and strategies.
5. Supervise and provide written instructions to Physical Therapy Assistants and Home Health Aides, ensuring proper documentation and performance.
6. Document patient care accurately and promptly, including observations, interventions, and evaluations.
7. Report patient progress to physicians and team members, and submit required reports and summaries.

Required Experience/Skills:

1. Degree from an accredited Physical Therapy Program (approved by the APTA).
2. At least one year of physical therapy experience preferred.
3. Current and unrestricted Physical Therapy license.
4. Current CPR certification.
5. Strong organizational and communication skills.

Pay Range:

• $54.00 - $76.00 per visit/unit
• $85,400 - $117,500 per year (base pay)
