Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

Position Type: Per Diem, Home Health Visits
Location: Chester County, senior living office

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) with at least one year of clinical experience to provide home health visits for adult and geriatric clients. The role involves working as part of a multidisciplinary team to deliver rehabilitative and skilled nursing care, enabling clients to remain at home.

Responsibilities

1. Make home visits to clients within designated geographic areas.
2. Conduct diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans with interventions like manual therapy, gait training, and balance exercises.
5. Assess and update care plans regularly, collaborating with nursing staff.
6. Educate clients and their families on rehabilitative activities to promote health and independence.
7. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications

• Current Pennsylvania Physical Therapist (PT) license.
• Minimum of one year recent clinical experience as a PT.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
