Job Summary

The Clinical Triage Specialist (CTS) (RN) - Access Center will provide compassionate patient care, serving as a clinical resource for the team. The CTS-RN uses nursing judgment to answer and triage patient calls, assessing symptoms and guiding patients to appropriate care or self-care, following approved protocols.

Responsibilities

1. Answer and prioritize clinical triage calls, follow protocols, and coordinate services.
2. Verify and update patient demographic information in the electronic health record.
3. Serve as an escalation point for clinical issues and provide advice based on protocols.
4. Respond to electronic patient messages, prioritizing and managing patient needs.
5. Create a positive patient experience, resolving issues within role scope.
6. Meet productivity, schedule adherence, and quality standards.
7. Utilize resources to assess, advise, schedule, or refer patients appropriately.
8. Document symptoms, assessments, advice, and patient responses accurately.
9. Collaborate with other teams to address clinical concerns and coordinate care.
10. Perform other duties as assigned.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in Pennsylvania or seeking licensure through reciprocity.
• Minimum 2 years recent clinical experience in physician offices, home health, critical care, or emergency settings.
• Strong communication, compliance, growth, and problem-solving skills.
• Ability to work from home in accordance with policy.
