6 Degrees Search is seeking a dedicated Physical Therapist to provide home health care in Colorado. This full-time role focuses on patient-centered therapy delivered in patients' homes, with a emphasis on accurate assessments and individualized treatment plans.

The position requires a CAPTE-accredited Master’s degree, current state licensure, CPR certification, and the ability to travel to all client locations.

Formación

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy from a CAPTE-accredited program.
• Licensed to practice as a physical therapist within the state of operation.

Responsabilidades

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
• Create and implement tailored treatment plans addressing rehabilitation goals.
• Conduct therapeutic exercises and training to improve mobility and function.
• Educate patients and families about injury prevention and recovery journey.
• Monitor and document progress, adjusting plans as necessary to achieve goals.
• Collaborate with a multidisciplinary team to ensure holistic patient care.
• Maintain meticulous patient records in compliance with regulatory standards.

Qualifications:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
