As The Physical Therapist, You Will

• Evaluate and assess patients to determine their appropriateness for care.
• Utilize available tools and resources to develop patient-specific care plans with appropriate level and frequency of care.
• Communicate with the care team, physicians, payors, patients, and families to address care needs and fulfill the patient's care plan.
• Deliver high-quality skilled care and supervise the care of other team members.
• Document care delivery daily in our EMR system thoroughly.
• Contribute to a culture of caring through individual accountability and teamwork.
