Trilogy Home Healthcare is hiring a Physical Therapist to provide one-on-one home health visits across North Palm Beach and West Palm Beach coverage areas. The role supports patient mobility, strength, and balance, with goal-driven care plans and safety assessments in patients’ homes.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• 1+ year home health experience preferred.

Type: Full Time & PRN/Per Diem available

Location: North Palm Beach and West Palm Beach coverage

Pay: Full Time ($100,000 - $120,000) PRN/Per Diem ($65 - $120/visit)

What you bring:

• Graduate of an accredited PT program
• Current PT license in Florida
• Valid Florida driver's license, active vehicle insurance, and clear MVR
• 1+ year of experience required, Home Health experience preferred
