Job summary

Preferred Care Home Health in West Palm Beach, FL is seeking a Full-Time Physical Therapist (PT) to deliver home health services. You will evaluate patients, develop individualized treatment plans, and provide skilled therapy in patients’ homes with a focus on mobility and independence.

Responsibilities include educating patients and caregivers, monitoring progress, and ensuring thorough documentation, while coordinating with physicians and the care team.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active, unrestricted Physical Therapist license in good standing.
• Home health experience preferred but not required.
• Valid driver’s license and auto insurance.
• Strong clinical judgment, communication, and documentation skills.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and established care plans.
• Monitor patient progress and adjust interventions to support optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive equipment.
• Complete accurate and timely clinical documentation.
• Communicate changes in patient condition to physicians and the interdisciplinary care team.
• Provide clinical supervision to Physical Therapist Assistants (PTAs).
• Participate in case conferences, care planning, and ongoing clinical education.
