Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to provide in-home therapy across Naples, FL. You will evaluate patients, develop treatment plans, and implement individualized protocols to restore function and independence.

The role requires collaboration with families and care teams, adherence to physician orders, and travel to patient homes with a valid PT license and driver’s license. A supportive, patient-centered environment awaits.

Qualifications

• Licensed PT in the state of employment.
• Valid driver’s license required.
• Ability to travel to patient homes.
• Excellent communication with patients and families.

Responsibilities

• Evaluate and treat patients to improve mobility and daily functioning.
• Administer skilled physical therapy services per physician orders and care plans.
• Educate patients' families and health care teams.
• Work under supervision of the Branch Director and coordinate care in home settings.
