BAYADA at Inspira Home Health is seeking a Physical Therapist for a full-time home health position in the Salem County area, based in West Deptford, NJ. This role involves providing care through home visits across Salem County, NJ.

As a home care Physical Therapist, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests such as mobility, joint range, and balance assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual therapy, gait training, and balance exercises.
5. Assess and update care plans, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current NJ Physical Therapist license.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; experience with EMR preferred.
