BAYADA Home Health Care is currently seeking an experienced per-diem, part-time or full-time Physical Therapist, PT for an opportunity performing home health visits for our office located in Cape May. This office services adult and geriatric clients on a per-visit basis throughout Cape May County.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise the care plan regularly, collaborating with nursing staff as needed.
5. Educate clients and family members on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touch pad tablet.
7. Implement wound care interventions using evidence-based dressings and topical agents to prevent and promote wound healing.

Qualifications:

• Current New Jersey Physical Therapist (PT) license.
• Graduation from an APTA-approved program or equivalent, as evidenced by diploma or transcript.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
