Pride Health is seeking a Physical Therapist (Home Health) to support a client facility in Winter Haven, Florida. The role covers a Monday–Friday schedule from 8:00 AM to 4:30 PM on a local contract.
The ideal candidate holds a Master’s degree, an active Physical Therapist license, CPR certification, and at least one year of geriatric home health experience; HomeCare HomeBase (HCHB) experience is a plus.

Pay Range: $52.00 - $56.00/hour (W-2) Plus Mileage reimbursement
Mileage Reimbursement Rate: $0.49 mile
