Responsibilities of a Home Health Physical Therapist include:

• Planning and administering prescribed skilled physical therapy treatments for patients with injuries, illnesses, or disabilities to maximize physical function.
• Testing and screening patients’ physical strengths to assist physicians in evaluating their level of function, and recording findings to develop or adjust treatment plans.
• Developing and implementing rehabilitation programs aligned with physicians’ plans and patient goals, adjusting treatments as needed.
• Collaborating with physicians and the clinical team to develop, implement, and revise therapy plans.
• Providing instructions and supervision to Physical Therapy Assistants and Home Health Aides, ensuring documentation accuracy and staff performance.
• Documenting patient care observations, interventions, and evaluations accurately and promptly, including interim orders and treatment updates.
• Reporting patient progress to physicians, managers, and families, and submitting required documentation such as evaluations and discharge summaries.

Required Experience and Skills:

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Preferred minimum of one year of physical therapy experience.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.

Pay Range:

• $65.00 - $91.00 per visit/unit
• $10,500 - $141,000 per year base pay

Scheduled Weekly Hours: 40
