Job summary

Jethro Health System seeks a Physical Therapist to join our caring team in Massachusetts. You will build trusted therapist-patient relationships and provide assessments to guide functional goals within a collaborative care model.

Responsibilities include performing initial and ongoing assessments, developing plans of care with physicians, supervising aides, documenting progress, and educating patients and families about safe use of adaptive equipment.

Qualifications

• Must hold current MA Physical Therapy license.
• Experience in acute care or home health is preferred but not required.
• Maintain strict confidentiality of protected information.
• Ability to solve problems independently through analytical thinking.
• Ability to read, interpret reports, directives, and regulations.
• Strong writing and communication skills with diverse populations.

Responsibilities

• Perform initial and ongoing assessments to determine functional level.
• Develop and revise plan of care with physician and care team.
• Manage an assigned caseload with evaluation and treatment.
• Prepare progress notes and evaluate outcomes of care.
• Supervise therapy assistants and home health aides as appropriate.

Physical Therapist (PT) Duties and Responsibilities:

• Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
• Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
• Develop and revise the plan of care in consultation with the physician and other care team members.
• Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
• Preparing clinical progress notes and evaluating outcomes.
• Supervising therapy assistants and home health aides as appropriate.
• Educate patients and/or families about proper safe use and recommend methods for the installation of adaptive equipment.

Physical Therapist (PT) Qualifications and Skills:

• Currently licensed by the Massachusetts Board of Physical Therapy (PT) Examiners.
• Experience in acute care settings or Home Health is preferred but not required.
• Able to maintain strict confidentiality about protected and sensitive information.
• Must be able to solve problems independently and confidently by applying analytical and logical thinking.
• Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
• Able to write reports and correspondence at a professional level.
• Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
