Job summary

Innovive Health seeks a compassionate Physical Therapist to join our home health team, delivering in-home PT to help patients regain independence.

You will conduct evaluations, start-of-care visits, follow-ups, and discharge planning, develop personalized plans, and educate patients and caregivers.

Travel to patient homes is required; MA licensure, CPR, and a valid driver’s license are needed; a collaborative, autonomous role with growth opportunities.

Qualifications

  • Must be a licensed Massachusetts PT.
  • Graduated from an accredited Physical Therapy program.
  • Current CPR certification.
  • Valid driver’s license with reliable transportation.
  • Home health experience required.
  • Strong communication, organization, and clinical decision-making skills.

Responsibilities

  • Provide in-home physical therapy services including evaluations, initial visits, follow-ups, recertifications, OASIS assessments, and discharges.
  • Develop and implement personalized treatment plans based on patient needs and physician orders.
  • Guide patients through therapeutic exercises, gait training, balance and mobility activities.
  • Educate patients and caregivers on safety, fall prevention, and daily living strategies.
  • Monitor patient progress and adjust treatment plans for optimal outcomes.
  • Assess vital signs as needed and communicate concerns to the care team.
  • Recommend and educate on the use of assistive devices and durable medical equipment (DME).
  • Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians.
  • Complete timely and accurate documentation.
  • Support discharge planning and continuity of care.

What You’ll Do

What We’re Looking For
  • Licensed Physical Therapist in the state of Massachusetts
  • Degree from an accredited Physical Therapy program
  • CPR certification (current)
  • Valid driver’s license and reliable transportation
  • Home health experience required
  • Strong communication, organization, and clinical decision-making skills
Preferred
  • At least 1 year of clinical experience

Work Environment & Physical Requirements
