Join to apply for the Physical Therapist, Home Health role at BAYADA Home Health Care.

VCU Health at Home by BAYADA is seeking an experienced Physical Therapist (PT) for a full-time position providing home health visits in North of the River, Richmond, VA. This role involves working as part of a multidisciplinary team to deliver rehabilitative care that allows clients to remain at home.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Performing diagnostic tests such as mobility, strength, balance, and safety assessments.
3. Implementing wound care interventions using evidence-based dressings and topical agents.
4. Developing and updating individualized care plans, including manual therapy, gait training, and balance interventions.
5. Assessing and revising care plans and collaborating with nursing staff.
6. Educating clients and families on rehabilitative activities to promote health and safety.
7. Documenting all client interactions and interventions using electronic records.

Qualifications include:

1. A current VA Physical Therapist license.
2. Graduation from an accredited physical therapy program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal and organizational skills.
5. Proficiency with electronic medical records; prior EMR experience preferred.
