Job description

Title : Physical Therapist (Home & Community-based Services)

Wages : $73.25/hour

Type : Part-time/Contract

Hours : Varies by week, flexible schedule

We are seeking a licensed Physical Therapist to provide contract-based services under the Home and Community-based Services (HCS) program. This position involves working with two clients with intellectual and developmental disabilities (IDD) to deliver skilled physical therapy in their home and community settings. The therapist will assess, plan, and implement therapy goals focused on increasing independence, improving mobility, and enhancing overall physical functioning.

This is a contract role with flexible scheduling, though hours will vary from week to week depending on client needs.

Responsibilities:
  • Conduct evaluations and develop individualized treatment plans

  • Provide in-home and community-based physical therapy sessions

  • Collaborate with interdisciplinary teams and family members

  • Maintain accurate and timely documentation in accordance with HCS guidelines

Qualifications:
  • Education : Graduate of an accredited Physical Therapy program (Doctorate or Master's degree preferred)

  • Licensure : Active Physical Therapist license in the state where services are provided

  • Experience :

    • At least 1 year of experience working in IDD, geriatrics, pediatrics, or home health preferred

    • Experience with HCS or Medicaid waiver programs is highly desirable

  • Certifications : Current CPR certification

Additional Requirements:
  • Strong communication and documentation skills

  • Ability to manage a flexible and changing caseload

  • Reliable transportation, valid driver's license, and auto insurance

  • Must pass all background and credentialing checks required by the HCS program
