Qualifications

  • Must possess an Oregon Physical Therapist License upon hire.
  • 1 year clinical experience in Home Health or healthcare setting.
  • Within 30 days of hire: National Provider BLS from American Heart Association.

Physical Therapist- Providence Home Services Oregon


Schedule: Full time, Days, Monday- Friday (8:00am - 4:30pm)


Responsible for East Portland area


Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.

Required Qualifications:



  • Bachelor's Degree From an accredited Physical Therapy Program. Or

  • Master's Degree From an accredited Physical Therapy Program. Or

  • D.P.T. From an accredited Physical Therapy Program.

  • Within 30 days of hire: National Provider BLS - American Heart Association

  • Upon hire: Oregon Physical Therapist License

  • 1year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

  • Washington Physical Therapist License (Vendor Managed) - if practicing within this state

About the Team


Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.


HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
