Physical Therapist – Home Health at Saint Joseph Medical Center

Schedule: Full time Day shift

Location Coverage: will service around Encino, Tarzana, West Van Nuys, Granda Hills, Chatsworth, Northridge

Pay Structure: PPV (Pay Per Visit)

Job description:

Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence.

Required Qualifications:

• Degree in Physical Therapy from an approved program according to Medicare guidelines.
• California Physical Therapist License upon hire.
• National Provider BLS - American Heart Association within 30 days of hire.
• 2 years - Experience in acute care, rehabilitation, or home care.

Preferred Qualifications:

• Home care experience.
