Adventist Health in Bay City, OR is seeking a licensed Physical Therapist to lead and participate in individualized rehabilitation programs. The role focuses on restoring physical, social, and mental function through evidence-based treatment within an interdisciplinary team.

You will evaluate patients, develop treatment plans, document progress, and supervise assistants while adhering to department objectives and safety policies.

Formación

• Doctor of Physical Therapy (DPT) degree required; only if license obtained after Jan 2016; or foreign equivalent.
• Physical Therapy licensure in all applicable states required.
• CPR/BLS certification required
• Valid driver's license and must be at least 21 years old

Job Summary:

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Job Requirements:

Education and Work Experience:

• Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.

Licenses/Certifications:

• Physical Therapy licensure in all applicable states of practice: Required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required
• Valid Driver's License (DL) and must be at least 21 years of age or older: Required

Essential Functions:

• Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
