Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time or part-time opportunity performing home health visits throughout the West Philadelphia and Fairmount areas.

This office services adult and geriatric clients on a per-visit basis. A minimum of one year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care to remain at home.

Responsibilities include:

1. Making home visits to clients in designated geographic areas.
2. Performing diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments.
3. Implementing integumentary protective interventions to prevent wound formation and promote wound healing using evidence-based dressings and topical agents.
4. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
5. Assessing and revising care plans regularly and collaborating with nursing staff as part of the multidisciplinary team.
6. Educating clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
7. Documenting observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications:

1. Current Pennsylvania Physical Therapist (PT) license.
2. At least one year of recent work experience as a PT.
3. Graduation from an American Physical Therapy Association-approved program or equivalent.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; prior EMR experience preferred.
