Location: Team Orange: This position involves conducting patient home visits in the Downtown Orlando, Pine Hills, West Orlando, Ocoee, Winter Garden, Windermere areas, including but not limited to zip codes 34760, 34787, 34734, 34786, 34761, 32818, 32835, 32819, 32811, 32808, 32810, 32714, 32804, 32805, 32839, 32806, 32801.

The role you’ll contribute:

The Home Health Physical Therapist (PT) Care Manager coordinates and directs services based on individual patient needs. Responsibilities include independent management of the home health patient population, advanced assessment, teaching, decision-making, ensuring appropriate referrals, interdisciplinary conferencing, and documentation. The PT evaluates patients for home health appropriateness, develops care plans with physicians, educates patients and families, and monitors outcomes. The PT also participates in performance improvement initiatives and standard of care enhancements.

• Manage a caseload of home health patients requiring physical therapy, including assessment, planning, implementation, and evaluation.
• Prioritize and adapt the care plan to meet changing patient needs, optimizing visit utilization.
• Conduct functional evaluations measuring activities of daily living and cognitive factors, documenting results.
• Develop patient-specific, realistic, measurable goals in collaboration with patients, families, and physicians, considering physical, cultural, psychosocial, spiritual, age, and educational needs.
• Reassess patients every 30 days, evaluate therapy effectiveness, and document outcomes.
• Record observations and report patient responses and condition changes to physicians.

Qualifications:

• Master's Degree in Physical Therapy
• At least 1 year of relevant clinical physical therapy experience

Preferred qualifications:

• Doctorate Degree
• Recent experience in a Medicare-certified home health agency as a care manager
• Certification as Oasis Specialist-Clinical (COSC)
