BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for part-time or per diem home health visits in the Lexington, VA area, serving adult and geriatric clients.

Requirements include at least one year of clinical experience as a licensed Physical Therapist. The role involves providing rehabilitative care and skilled nursing to enable clients to remain at home, working as part of a multidisciplinary team.

Responsibilities include:

• Making home visits within designated territories.
• Performing diagnostic tests, assessments, and developing individualized care plans.
• Implementing wound care interventions and educating clients and families.
• Documenting care activities accurately using electronic devices.

Qualifications:

• Current VA Physical Therapist license.
• Minimum one year of recent experience as a PT.
• Graduation from an accredited program.
• Ability to work independently, with strong interpersonal and organizational skills.
• Computer proficiency; EMR experience preferred.
