The role you’ll contribute:

The Home Health Physical Therapist (PT) Care Manager is a professional therapist who coordinates and directs the Home Health patient's services based on individual patient needs. The PT Care Manager is responsible for independent management of the Home Health patient population, requiring advanced assessment, teaching, and decision-making skills. Responsibilities include ensuring appropriate referrals, regular interdisciplinary conferencing, and proper documentation. The PT applies relevant knowledge and experience to new patient populations and cares for a caseload of home health patients requiring therapy as the primary service. This involves evaluating patients for home health appropriateness, developing plans in conjunction with physicians, educating patients and families, and monitoring progress to achieve patient/family goals and outcomes. The PT also identifies performance improvement opportunities and assists in implementing care standards and program improvements.

• Coordinate and direct care for a caseload of home patients requiring physical therapy as the primary skill. Provide comprehensive assessment, planning, implementation, and evaluation.
• Set priorities for the caseload, adapting to changing needs, and optimize scheduling for productivity and best practice visit utilization.
• Perform evaluations assessing function through objective measures of activities of daily living, documenting results accurately.
• Formulate patient-specific plans of care in collaboration with patients, families, and physicians, establishing realistic, measurable, patient-centered goals considering physical, cultural, psychosocial, spiritual, age-specific, and educational needs.
