Job summary

Freedom At Home is seeking a compassionate Physical Therapist to join our home-based care team in Michigan. You'll assess patients in their homes, create individualized treatment plans, and collaborate with physicians, nurses, and caregivers to deliver high-quality care.

The role emphasizes flexible scheduling, per-visit compensation, paid mileage, and a supportive leadership team, with a mission to help patients regain independence and improve quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed PT in Michigan.
• CPR certification required.
• Minimum of 2 years of PT experience preferred.
• Home Health experience is a plus but not required.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled physical therapy services
• Educate patients and caregivers on exercises, safety, and mobility
• Complete OASIS and clinical documentation in a timely manner
• Collaborate with physicians, nurses, and other healthcare professionals
• Monitor patient progress and adjust treatment plans as needed
• Promote safe, high-quality patient care and excellent outcomes
