Job Description Summary

The physical therapist is responsible for providing physical therapy services.

Essential Job Functions/Responsibilities

• Providing physical therapy services according to a written physician’s plan of care, including assessment and evaluation of therapeutic, rehabilitative, and functional status, and participation in developing the care plan. Evaluating home environment.
• Assessing muscle strength, mobility, gait, ROM, and rehab potential.
• Directing physical therapy treatments.
• Instructing patients and families/caregivers on the use and care of therapeutic appliances.
• Determining priority needs for physical therapy.
• Reporting patient reactions to treatment or changes in condition to the physician.
• Initiating therapy programs and instructing personnel and family/caregivers, including participation in case conferences.
• Training patients in the use of prosthetic devices.
• Supervising homemaker/home health aides involved in patient rehabilitation.
• Identifying needs for other home health services and making referrals as necessary.
• Preparing and submitting clinical and progress summaries based on goal attainment.
• Participating in discharge planning.
• Providing physical therapy consultations to families/caregivers as needed.
• Providing in-service education to nursing agency personnel.
• Participating in peer consultation processes.

Position Qualifications

• Degree from an accredited baccalaureate or master's program in physical therapy.
• State licensure to practice as a physical therapist, if applicable.
• At least two years of relevant experience, with community/home health preferred.
• Good verbal and written communication and organizational skills.
• Current CPR certification.
