Organization: SCAH-Sutter Care at Home - Bay

Position Overview: $10,000 sign-on bonus available to external candidates - in accordance with Sutter Health policy + excellent benefits!

Primary coverage areas: San Jose, Santa Clara & surrounding

Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Demonstrates advanced knowledge and practice level competencies, and is a recognized clinical or workflow subject matter expert. Responsible for developing key workflows and clinical protocols or guidelines for the department. Serves as a resource in education, training, and mentoring of staff and students. Leads staff education programs and coordinates student programs. Adheres to all relevant regulations, policies, and procedures to ensure privacy and safety while delivering patient care.

Qualifications

• Education: Graduate of an accredited physical therapy school
• Certification & Licensure: PT-Physical Therapist, BLS-Basic Life Support Healthcare Provider
• Department Required Certification & Licensure: California Drivers Class C, Auto Insurance
• Experience: 5 years of relevant recent experience

• Ability to work in demanding environments, organize priorities, and meet deadlines

Work Schedule and Benefits

• Shift: Days, Full Time, Monday - Friday, Variable, Rotating Weekends
• Hours: 8 hours per day, 40 hours per week

Employees may handle hazardous drugs as per NIOSH and USP 800 guidelines. Sutter Health is an equal opportunity employer. Pay rate: $71.33 to $89.16/hour, with variation based on location and experience.
