Base pay range

$85,400.00/yr - $117,500.00/yr

Role Overview

As a Home Health Physical Therapist, you will:

• Plan and administer prescribed skilled physical therapy treatments for patients with various injuries, illnesses, and disabilities to maximize their physical function.
• Assess patients' physical strengths to aid physicians in evaluating their functional levels and develop treatment plans accordingly.
• Develop and implement rehabilitation programs aligned with physician’s plans and patient goals, adjusting treatments as needed.
• Collaborate with physicians and the clinical team to refine therapy plans and ensure comprehensive patient care.
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides, providing clear instructions and monitoring documentation accuracy.
• Document patient care activities thoroughly and promptly, including observations, interventions, and evaluations, and ensure compliance with orders and protocols.
• Communicate patient progress to physicians, care teams, patients, and families, and prepare necessary reports and summaries.

Required Experience and Skills

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• At least one year of physical therapy experience preferred
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills

Additional Compensation Details

Pay varies based on location and experience, with estimates including:

• $54.00 - $76.00 per visit/unit
• $85,400 - $117,500 per year as base pay
