Salary Range: $85,000-$100,000

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient care needs related to mobility, endurance, pain, and other medical conditions. Based on this, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate patient's functional status and therapy needs; assist physicians in developing the plan of care.
• Monitor and report patient responses and condition changes to the healthcare team.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Educate patients, families, and healthcare team members about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
