General Summary of Position

MedStar Health is seeking a PT (Physical Therapy) Homecare Clinician PV (Per Visit) to join our team! As a PT Homecare Clinician PV, you will evaluate and provide physical therapy treatment in accordance with agency standards, laws, regulations, and other regulatory requirements in Maryland, District of Columbia, or Virginia.

Primary Duties:

1. Analyze and document patients' and families' responses to interventions, evaluate progress, and revise the plan of care accordingly.
2. Identify and modify underlying factors impeding progress, and recommend updates to the care plan.
3. Establish and document teaching plans for patients and families based on their knowledge, diagnosis, treatment, and resources.
4. Communicate effectively with patients, families, healthcare team members, and supervisors.
5. Implement and modify physical therapy treatment plans, including assessments like goniometry, muscle strength, and sensory testing.
6. Independently implement and document care plans for routine to complex problems, ensuring continuity of care.
7. Maintain clinical records, collect data, and submit reports within specified timeframes.
8. Participate in case conferences, community outreach, and educational programs.
9. Perform assessments, recommend equipment or home modifications, and identify discharge planning needs.
10. Perform procedures such as therapeutic exercises, gait training, heat/cold application, and ultrasound.

Qualifications:

• Bachelor's degree in physical therapy from an accredited school.
• 1-2 years of clinical experience.
• License to practice Physical Therapy in DC, MD, or VA.
• CPR certification.
• Valid State Driver’s License with dependable vehicle.

This position has a hiring range of $72,758 - $130,041.
