General Summary of Position

MedStar Health is seeking a PT (Physical Therapy) Homecare Clinician PV (Per Visit) to join our team! As a PT Homecare Clinician PV, you will evaluate and provide physical therapy treatment in accordance with agency standards, state laws, and regulatory requirements in Maryland, District of Columbia, or Virginia.

Primary Duties:
  1. Analyze and document patient and family responses to interventions; evaluate progress; revise plans of care; and make recommendations for updates based on interventions and resources.
  2. Establish and document teaching plans for patients and families, considering their knowledge level, diagnosis, and treatment.
  3. Communicate effectively with patients, families, supervisors, peers, and other health team members.
  4. Set visit schedules and inform patients, families, and team supervisors.
  5. Implement and modify effective physical therapy treatment plans, including assessments like goniometry and muscle strength testing, using appropriate tools.
  6. Independently develop and document care plans for patients with routine to complex problems, utilizing agency and community resources.
  7. Perform therapy interventions consistent with protocols and knowledge of home care supplies and equipment.
  8. Organize patient assignments efficiently, seek assistance for complex cases, maintain clinical records, and submit reports timely.
  9. Participate in case conferences, include patients and families in goal setting, and engage in community outreach and educational activities.
  10. Perform assessments to determine rehabilitation needs, recommend adaptive equipment or home modifications, and identify discharge planning needs.
  11. Perform procedures such as therapeutic exercises, mobility training, gait training, and modalities like heat/cold application and ultrasound.
Qualifications:
  • Bachelor's degree in physical therapy from an accredited school.
  • 1-2 years of clinical experience.
  • License to practice Physical Therapy in DC, Maryland, or Virginia.
  • CPR certification.
  • Valid State Driver's License and a dependable vehicle.

This position offers a hiring range of $72,758 - $130,041.
