Job description

The *Home Care Physical Therapist* is responsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients. The PT is responsible to supervise Physical Therapy Assistants and cosign their documentation when required. Physical therapist supports organizations mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
