Job description

MedStar Health is seeking a part-time (Per Visit) Physical Therapy (PT) Homecare Clinician to join our team!

As a PT Homecare Clinician PV, you will evaluate and treat patients in accordance with agency standards, state laws, and regulatory requirements in Maryland, District of Columbia, or Virginia.

Primary Duties:
  • Assess and document patient and family responses to interventions, evaluate progress, and revise care plans accordingly.
  • Identify and modify factors affecting progress, and recommend updates to care plans based on interventions and available resources.
  • Develop and document patient and family education plans tailored to their knowledge, diagnosis, and treatment.
  • Communicate effectively with patients, families, and healthcare team members.
  • Establish visit schedules and inform patients and team members accordingly.
  • Implement and adapt physical therapy treatment plans, including assessments like goniometry and muscle testing, using appropriate tools.
  • Manage care plans independently for routine to complex cases, ensuring interventions meet standards of care and utilize community resources.
  • Maintain clinical records, collect data, and prepare activity reports, submitting documentation promptly.
  • Participate in case conferences, multidisciplinary meetings, and community outreach efforts.
  • Perform patient assessments, recommend adaptive equipment and home modifications, and identify discharge needs.
  • Perform procedures such as therapeutic exercises, gait training, and modalities like ultrasound.
Qualifications:
  • Bachelor's degree in Physical Therapy from an accredited institution.
  • 1-2 years of clinical experience.
  • Valid license to practice in DC, Maryland, or Virginia.
  • CPR certification.
  • Valid driver's license and dependable vehicle.
