MedStar Health is looking for a PT (Physical Therapy) Homecare Clinician PV (Per Visit) to join our team! As a PT Homecare Clinician PV, you will provide evaluation and physical therapy treatment in accordance with agency standards, laws, regulations, and other regulatory requirements in Maryland, District of Columbia, or Virginia. Primary Duties
  1. Analyze and document patients and families' responses to interventions.
  2. Evaluate measurable progress toward goals and revise the plan of care.
  3. Identify and modify underlying factors impeding progress.
  4. Make recommendations for updates in the plan of care based on interventions and resources.
  5. Establish and document teaching plans for patients and families.
  6. Communicate respectfully with patients, families, supervisors, peers, and other health team members.
  7. Establish visit schedules and inform patients, families, and supervisors.
  8. Implement and modify physical therapy treatment plans with the patient, family/caregiver, physician, and healthcare team.
  9. Conduct evaluations including interviews, observations, palpation, and specialized assessments like goniometry, muscle strength, and sensory testing.
  10. Use appropriate tools and senses, or prosthetics, to perform assessments.
  11. Implement and document care plans for routine to complex problems, facilitating continuity of care.
  12. Perform therapy interventions consistent with home care protocols and standard supplies and equipment.
  13. Organize patient assignments efficiently, seeking assistance for complex issues.
  14. Maintain clinical records, collect data, and prepare reports timely.
  15. Participate in case conferences, community outreach, and educational programs.
  16. Perform assessments to determine habilitation/rehabilitation needs and recommend equipment or modifications.
  17. Identify discharge planning needs and appropriate community referrals.
  18. Interpret clinical data to anticipate patient problems.
  19. Perform procedures like therapeutic exercises, mobility training, gait training, heat/cold application, and ultrasound.
Qualifications
  • Bachelor's degree in physical therapy from an accredited school.
  • 1-2 years of clinical experience.
  • License to practice Physical Therapy in DC, Maryland, or Virginia.
  • CPR certification.
  • Valid State Driver's License with a dependable vehicle.

This position has a hiring range of $72,758 - $130,041.
