Community Health Systems is seeking a Physical Therapist to evaluate, plan, and administer therapeutic procedures in rehabilitation as prescribed by a physician. You will set individualized goals, provide treatments, and reassess progress, documenting all findings accurately.

You will collaborate with referring physicians and the interprofessional team, educating patients and families on treatment plans and post-treatment care to support recovery.

Qualifications

• Strong knowledge of physical therapy principles and safety standards.
• Ability to perform comprehensive evaluations and develop effective treatment plans.
• Proficiency in using therapy equipment and documentation/billing systems.

Responsibilities

• Conduct thorough PT evaluations to assess patient needs and develop individualized treatment plans.
• Administer safe and effective therapy treatments with proper documentation.
• Reassess and adjust treatment plans as patient progress dictates.
• Provide clear documentation of evaluations, plans, progress notes, and billing information.
• Educate patients and families on treatment plans and home exercise programs.
• Operate therapy equipment safely and maintain a clean, safe environment for patients and staff.
• Collaborate with physicians and interdisciplinary team to ensure continuity of care.
