Job summary

Qualifications

• PT license in the state of employment required.
• BCLS (Basic Life Support) certification within 30 days of hire required.

Responsibilities

• Conduct thorough PT evaluations to assess patient needs and develop individualized treatment plans.
• Administer safe and effective therapy treatments with proper documentation.
• Reassess and adjust treatment plans to match patient progress and needs.
• Document evaluations, treatment plans, progress notes, and billing information clearly and accurately.
• Educate patients and families on treatment plans and post-treatment care to support recovery.
• Collaborate with physicians and the care team to ensure continuity of care.

Job Summary

Essential Functions

• Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
• Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
• Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
• Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
• Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
• Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
• Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
• Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
• Performs other duties as assigned.
