Job summary

Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to assess and rehabilitate patients in a subacute setting. The PT will design and implement treatment plans, set achievable goals, and document patient progress under physician direction.

Responsibilities include supervising Physical Therapy Assistants, documenting evaluations, progress notes and discharge summaries, while collaborating with the broader care team to ensure high-quality, patient-centered outcomes.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid state license/registration as a Physical Therapist.
• Oversees Physical Therapy Assistants.
• Long-term/geriatric care experience preferred.
• Strong documentation writing ability.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer a treatment program with set goals.
• Document patient progress and educate patients and families.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain professionalism and adhere to schedules and confidentiality.
