Job summary

The Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist (PT) to evaluate and treat patients requiring rehabilitation services in a subacute facility. The PT will develop treatment plans, guide rehabilitation goals, and document patient progress under physician direction.

Responsibilities include supervising Physical Therapy Assistants, performing evaluations, weekly progress notes, and discharge documentation, while ensuring confidentiality and compliance with

Qualifications

• Graduate from an accredited Physical Therapy program.
• Licensed and registered to practice in the state.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation writing and charting.
• Knowledge of PPS scheduling and Case Mix Index is a plus.
• Commitment to confidentiality and residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment program with goals based on patient capacity and tolerance.
• Document and record the patient’s condition and educate patients and families in PT methods.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate professional conduct, punctuality, and adherence to hours.
