Job summary

Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to assess and treat patients requiring rehabilitation, with a focus on geriatric care. The role includes supervising PT Assistants, documenting progress, and coordinating with a multidisciplinary team to achieve optimal patient outcomes.

The ideal candidate will have an accredited Physical Therapy degree, active license, and strong clinical skills in long-term care settings, along with commitment to confidentiality

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Experience with evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Document and educate patients and families in physical therapy methods.
• Ensure timely and accurate discharge documentation and progress notes.
