Job summary

Sequoia Transitional Care in Porterville, CA is seeking a Staff Physical Therapist to evaluate and treat residents, collaborate with the healthcare team, and contribute to documentation and education. Both full-time and part-time positions are available, with a starting pay of $60–$65 per hour depending on experience.

Qualifications

• Valid California Physical Therapy License (Required).
• Current CPR Certification (Required).
• Experience in geriatrics or rehabilitation preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to relate effectively with patients, families, and interdisciplinary team.

Responsibilities

• Evaluate patients within 24 hours of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Deliver treatment in accordance with physician orders and PT best practices.
• Supervise Physical Therapy Assistants in accordance with state regulations.
• Maintain detailed, accurate, and timely documentation of all patient care.
• Participate in interdisciplinary meetings including Care Conferences and Utilization Reviews.
• Provide caregiver education and training for post-discharge care.
• Recommend and assist with ordering of durable medical equipment (DME).
• Ensure all department equipment is maintained in good working condition.
• Conduct in-service education for facility staff.
