Job summary

Sequoia Transitional Care in Porterville, CA is hiring a Staff Physical Therapist for full-time or part-time work. The PT will evaluate, treat, document, and collaborate with the healthcare team to provide high-quality rehab services in a skilled nursing setting.

Qualifications

• Valid California Physical Therapy License (Required)
• Current CPR Certification (Required)
• Experience in geriatrics or rehabilitation medicine preferred
• Strong communication, documentation, and interpersonal skills
• Ability to relate effectively with patients, families, and interdisciplinary team
• Reliable transportation and/or ability to relocate to Porterville, CA

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of physician referral
• Develop and implement individualized treatment plans with physician approval
• Deliver treatment in accordance with physician orders and PT best practices
• Supervise Physical Therapy Assistants in accordance with state regulations
• Maintain detailed, accurate, and timely documentation of all patient care
• Participate in interdisciplinary meetings including Care Conferences and Utilization Reviews
• Provide caregiver education and training for post-discharge care
• Recommend and assist with ordering of durable medical equipment (DME)
• Ensure all department equipment is maintained in good working condition
• Conduct in-service education for facility staff
