Qualifications

• Valid California Physical Therapy License (Required).
• Current CPR Certification (Required).
• Geriatrics experience or rehabilitation medicine preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to relate effectively with patients, families, and interdisciplinary team.

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Deliver treatment in accordance with physician orders and PT best practices.
• Supervise Physical Therapy Assistants in accordance with state regulations.
• Maintain detailed, accurate, and timely documentation of all patient care.
• Participate in interdisciplinary meetings including Care Conferences and Utilization Reviews.
• Provide caregiver education and training for post-discharge care.
• Recommend and assist with ordering of durable medical equipment (DME).
• Ensure all department equipment is maintained in good working condition.
• Conduct in-service education for facility staff.
